The Local Organizing Committee will be responsible for issuing visa application forms to those and their family members who have registered through our web page. If you need the visa application forms, please fill the following forms and mail to dangl@ihep.ac.cn. Invitation letters and visa application forms (if required) will be issued starting from June, 2009, depending on the date of your registration.
